Pharmacodynamics and pharmacokinetics

The basis of the multienzyme drug Mezim is pancreatin . Wikipedia states that the substance is a powder that is obtained from the pork pancreas (PG) by extraction.

The substance contains excretory enzymes of the pancreas (amylase, lipase, trypsin and chymotrypsin), which are necessary for the digestion of proteins, carbohydrates and fats.

Pancreatin is not absorbed, but is excreted with the contents of the intestine. Most of the dose taken is broken down and denatured in the digestive canal under the influence of bacteria and digestive juices.

The decisive factors for the effectiveness of the drug are the lipolytic activity of pancreatin, as well as the proportion of trypsin, while amylase activity is important only in patients with cystic fibrosis .

The drugs Mezim and Mezim Forte have differences. The coating of Mezim Forte tablets is not enteric-soluble, which is why the lipolytic activity of the drug is inactivated even when the pH value of the gastric juice is below 4.

Mezim Forte 10000 and Mezim 20000 tablets are coated with a red coating that does not dissolve under the influence of gastric juice and thus protects pancreatic enzymes from inactivation.

Dissolution of the membrane and release of enzymes occurs only in a neutral or slightly alkaline environment of the small intestine.

Indications for use of Mezim

Indications for use Mezim Forte

The tablets are designed to improve digestion. Indications for their use are:

  • dyspepsia;
  • simultaneous consumption of fatty, difficult to digest, unusual foods;
  • flatulence associated with these disorders ;
  • functional disorders accompanied by accelerated transit of intestinal contents.

What do Mezim Forte 10000 tablets help with?

The drug is prescribed for:

  • diseases accompanied by digestive disorders due to insufficient secretion of essential enzymes from the pancreas;
  • conditions after simultaneous removal of sections of the small intestine and stomach;
  • functional disorders accompanied by accelerated transit of intestinal contents;
  • simultaneous consumption of fatty, difficult to digest, unusual foods; flatulence, intestinal disorders;
  • preparation for ultrasound or x-ray diagnostic studies.

When is Mezim 20000 prescribed?

Mezim 20000 is effective for:

  • insufficiency of exocrine function of the pancreas;
  • chronic inflammatory-dystrophic diseases of the liver, intestines, gall bladder, stomach;
  • conditions after irradiation or resection of the listed organs, if these conditions are accompanied by impaired digestion of food, diarrhea , flatulence ;
  • to eliminate the consequences of dietary errors and improve digestion in patients with normal pancreatic function;
  • preparing patients for ultrasound or x-ray diagnostic examinations;
  • functional gastrointestinal disorders: infectious intestinal diseases, irritable bowel syndrome .

Heartburn medications

If heartburn does not bother you often, once a week or less, you can take antacids and alginates .

Antacids

This group of drugs neutralizes hydrochloric acid produced by the cells of the gastric mucosa. Reducing acidity in the stomach helps to increase the tone of the lower esophageal sphincter.

Modern combination drugs that additionally have an enveloping and sometimes calming effect include:

  • Phosphalugel;
  • Almagel A;
  • Gastal;
  • Maalox;
  • Relzer.

The main thing is that these medications do not have rebound syndrome, like regular soda. When using it, the acidity first decreases, and after a few hours it increases again.

Alginates

  • Gaviscon;
  • Gaviscon Double Action.

Preparations from the alginate group form a gel film on the surface, which protects the mucous membrane from the action of acid and pepsin. Some drugs have a double effect, since they contain an antacid component.

H2-histamine receptor blockers

  • Ranitidine;
  • Famotidine;
  • Nizatidine.

They reduce the production of hydrochloric acid by the parietal cells of the stomach. The advantage of their action is that the secretion associated with food intake is suppressed to a lesser extent, which means the digestive processes will be affected to a lesser extent. At the first symptoms of heartburn, it is possible to take halved doses of H2-histamine receptor blockers. The effect can last up to 12 hours, and there is no need for additional antacid medications.

H2-histamine receptor blockers reduce the production of hydrochloric acid by the parietal cells of the stomach.

Proton pump inhibitors (PPIs)

  • Nexium;
  • Nolpaza;
  • Pariet;
  • Omez.

Proton pump inhibitors inhibit the final stage of hydrochloric acid formation in the parietal cells of the stomach. The maximum effect of administration develops on days 5-7 of administration. The secretion of hydrochloric acid is restored on the 2nd day after stopping the intake. With prolonged use of drugs for several years and a severe lack of hydrochloric acid in the stomach, pathogenic flora can develop. Drugs in this group do not have an immediate effect and can only be prescribed by a doctor after diagnosis.

Prokinetics

  • Ganaton;
  • Motilium;
  • Motilak.

If the symptoms of heartburn go along with impaired motor function of the stomach, the organ is sensitive to stretching, prokinetics help accelerate the evacuation of food into the small intestine. The propulsive motility of the stomach increases.

UDCA preparations

  • Ursosan.

Sometimes not only the contents of the stomach, but also the contents of the small intestine, such as bile, are thrown into the esophagus. This situation can arise in case of liver diseases, when drugs that reduce acidity in the stomach have already been used, there is no reflux of the acidic part, but there is irritation and heartburn in the esophagus. In this case, ursodeoxychoic acid (UDCA) preparations are recommended. They help the liver restore its function, normalize the formation and flow of bile, and can reduce the symptoms of heartburn. The course of application is from 2 weeks to 6 months.

Contraindications

Contraindications for the use of Mezim Forte:

  • hypersensitivity to pancreatin obtained from pig livestock, food additive E122 (synthetic food coloring azorubine), auxiliary components of tablets;
  • acute pancreatitis;
  • cases of exacerbation of chronic pancreatitis ;
  • obstructive intestinal obstruction (occurs when a mechanical obstruction appears).

For pancreatitis, occasional use of the drug is possible during the period of restorative dietary nutrition when the exacerbation subsides.

Conclusion

Self-medication can be practiced for mild, rare symptoms. For this purpose, drugs from the group of antacids and alginates are used, and a single dose of Ranitidine 75 mg is also possible. If heartburn bothers you more than once a week , you need to consult a doctor . Taking medications should not be uncontrolled. Self-administration should not last longer than 7-10 days. Otherwise, you may not notice other serious diseases in time, one of the symptoms of which may be heartburn.

Side effects

Side effects of Mezim Forte appear as:

  • immediate hypersensitivity reactions (sneezing, lacrimation, skin rashes, bronchospasm );
  • disorders of the digestive system, including nausea, diarrhea, abdominal pain, changes in bowel habits.

The additive E122 (azorubine varnish) contained in the tablets can also cause allergic reactions.

The annotation for Mezim Forte 10000 and Mezim 20000 indicates that in patients with cystic fibrosis , when using high doses of the drug, narrowing in the ileocecal region and in the ascending colon is sometimes possible.

Also, in patients with cystic fibrosis , the possibility of increased excretion of uric acid in the urine cannot be excluded, especially when taking high doses of pancreatin. To avoid the formation of uric acid stones in such patients, its concentration in the urine should be kept under control.

Contraindications and side effects

If the functioning of the pancreas is impaired, even after long-term use of Mezim, no side effects are observed. In rare cases, an allergic reaction occurs. Constipation, diarrhea and nausea rarely occur.

The use of Mezim is contraindicated in the following cases:

  • Intestinal obstruction;
  • Acute pancreatitis;
  • Sensitivity to the components of the drug.

There have been no cases of drug overdose in children. To prevent incorrect administration, the instructions are studied.

Directions for use and doses

The dose of Mezim® 20000 is set individually depending on the disease and severity of symptoms.

Unless otherwise indicated, adults should take 1-2 tablets of Mezim® 20000 with meals. The drug is taken orally without breaking or chewing the tablet (the effectiveness of Mezim® 20000 may decrease when chewed and pancreatic enzymes, when released, may irritate the oral mucosa), with a sufficient amount of liquid (for example, a glass of water).

It is not recommended to exceed the daily dose of pancreatic enzymes corresponding to 15,000-20,000 IU of lipase/kg of body weight.

Increasing the dose should only be done under the supervision of a physician, focusing on symptoms (for example, a decrease in steatorrhea and abdominal pain).

The duration of treatment is determined by the doctor and can vary from several days (in case of indigestion due to an error in diet) to several months and even years (if constant replacement therapy is necessary).

In children, the dosage is determined by the doctor.

Dose for children and adults with cystic fibrosis

In patients with cystic fibrosis, the dose should be adjusted based on the severity of symptoms of the disease, control of steatorrhea, and maintenance of adequate nutritional status (see section "Special Instructions").

The dose should not exceed 10,000 IU lipase/kg body weight per day.

In children, the dosage regimen and duration of treatment are determined by the doctor depending on the severity of digestive disorders and the composition of the diet, at the rate of 1000 IU of lipase/kg of child’s body weight for each meal for children under 4 years of age and 500 IU of lipase/kg of child’s body weight for each meals for children over 4 years old.

If there is no improvement after treatment, or symptoms worsen, or new symptoms appear, you should consult your doctor.

Use the drug only according to the indications, method of administration and in the doses indicated in the instructions.

If you experience side effects listed in the instructions or they get worse, or you notice any other side effects not listed in the instructions, tell your doctor.

Overdose

Symptoms

: hyperuricosuria and hyperuricemia (especially in patients with cystic fibrosis taking high doses of enzyme preparations).

Treatment

: symptomatic therapy, drug withdrawal.

Interaction with other drugs

It is possible to reduce the absorption of folic acid when used simultaneously with pancreatin preparations. The effect of oral hypoglycemic agents (such as acarbose, miglitol) may be reduced when used simultaneously with digestive enzyme preparations containing enzymes that break down carbohydrates (for example, amylase).

If you are using the above or other medications (including over-the-counter medications), consult your doctor before using Mezim® 20000.

special instructions

In case of acute pancreatitis or exacerbation of chronic pancreatitis (at the stage of attenuation of the exacerbation), during the period of restorative dietary nutrition, it is advisable to take the drug Mezim® 20000 against the background of existing and/or remaining exocrine pancreatic insufficiency.

In patients with cystic fibrosis, especially when taking high doses of pancreatin, hyperuricosuria may occur, so the concentration of uric acid in the urine should be monitored in this group of patients.

In the above group of patients, strictures in the ileocecal and ascending colon are described.

As a precautionary measure, if unusual abdominal discomfort occurs or changes in the nature of complaints, especially when taking more than 10,000 IU of lipase/kg body weight per day, a medical examination is necessary.

The drug Mezim® 20000 contains active enzymes that, when released in the oral cavity, for example during chewing, can damage the oral mucosa (including the formation of ulcers). Therefore, the tablet must be swallowed whole.

The drug contains lactose, so it is contraindicated in patients with hereditary galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome.

Precautionary measures for Mezim Forte 10000 and Mezim 20000

Intestinal obstruction is a known complication in patients with cystic fibrosis , so if symptoms reminiscent of this condition appear, you should be aware of possible intestinal strictures.

The drug contains active enzymes that can damage the oral mucosa, even to the point of ulceration, so the tablets must be swallowed whole without chewing.

Patients with congenital galactose intolerance, lactase deficiency and or impaired absorption of glucose/galactose are contraindicated to take the drug.

There is no need to limit activities related to driving a car or dangerous machinery.

Interactions with drugs

Pancreatin reduces the absorption of folic acid, so during long-term therapy it is necessary to ensure its additional supply. Miglitol and Acrabose, used in the treatment of diabetes, will not be able to effectively lower blood sugar when taken simultaneously with the drug Mezim forte.

Iron supplements are not fully absorbed through the intestinal walls while taking pancreatin. Antacids with active substances in the form of calcium carbonate or magnesium hydroxide reduce the effectiveness of the drug.

Which is better: Festal or Mezim?

The active substances of the drug Festal are pancreatin, hemicellulose and bile components. The medicine is available in the form of enteric tablets.

The protective shell reduces the aggressive effects of acid, and also helps ensure that the active substances enter and begin to act in the intestines, where the help of enzymes is needed.

However, there are also disadvantages - the pills, like Mezim tablets, are taken during lunch, that is, they enter the intestines with food, and not necessarily with the first portion. The coating with which Mezim tablets are coated begins to dissolve in the stomach, due to which food enters the intestines with enzymes.

In Festal, one of the components that enhances its effect is bile, which facilitates the digestion of fats. And this is basically very good, but this effect is strictly contraindicated in patients with cholelithiasis .

Mezim or Creon - which is better?

Creon has an active ingredient identical to Mezim, but in the drug Creon it is contained in a much higher concentration.

However, this is not the main difference between the drugs. First of all, they are distinguished by the form of release. Creon is available in the form of capsules filled with minimicrospheres.

The unique dosage form of the drug ensures its higher effectiveness (including for severe digestive disorders) - the microcomponents of the capsules are mixed almost evenly in the stomach with food, due to which the medicine enters the intestines with each serving of food, and the food is digested much more evenly.

It has been proven that the use of pancreatin in the form of capsules filled with microspheres is much more effective than the use of conventional tablets and mini-tablets: during treatment with Creon, the patient’s digestive function is restored more quickly and completely, and the duration of the relapse-free period is increased.

The downside of the drug is its high content of active components - concentrated accumulations of enzymes can irritate the intestines. In addition, it is known that the supply of enzymes in large quantities from the outside can provoke a situation when the pancreas stops producing its own enzymes.

High doses of pancreatin are effective in the treatment of severe pathologies, however, when it comes to treating pregnant women or children, the low content of enzymes in Mezim is its advantage over its analogue.

Composition and action

Mezim: tablet form

It is worth saying right away that Mezim is an enzyme preparation. The main component in it is pancreatin. Pancreatin is an enzyme that contains almost all the substances produced by the pancreas. It contains:

  • Lipase. This is a water-soluble enzyme that is involved in the digestion of fats. The main purpose of this enzyme is to break down lipids and ensure the delivery of fatty acids to cells.
  • Amylase. This is a substance that helps in the breakdown of carbohydrates. It breaks down starch into oligosaccharides. Thanks to this enzyme, you can feel a sweetish taste in your mouth if you chew potatoes or bread for a long time. This enzyme is also present in human saliva
  • Protease. This enzyme helps break down the peptide bond between amino acids

Due to the fact that Mezim contains precisely these enzymes, it perfectly helps to absorb fats, carbohydrates and proteins, which play an important role in the human body. If we talk about the principle of action of the drug, then it is worth saying the following.

It is recommended to take Mezim either with food or before eating it. This must be done so that the pancreas has time to react and begin producing the necessary enzymes.

After the pill has been taken, all the enzymes necessary for digestion begin to be produced. In this way, the body is relieved of enormous stress when food stagnates in the stomach, which leads to nausea, bloating and vomiting. Sometimes Mezim is recommended to be taken before each meal in order to restore the function of the gland.

Mezim is a very good drug, the action of which is based on helping in the production of all the enzymes a person needs for the normal digestion process.

Mezim during pregnancy and lactation

Is it possible to drink Mezim Forte during pregnancy?

Officially, there are no contraindications to the use of Mezim during pregnancy , but women still doubt whether pregnant women can take these pills and whether it will harm the unborn child.

The manufacturer indicates that Mezim tablets contain enzymes identical to those produced in the human body. Lipase acts on fats, amylase breaks down carbohydrates, and proteases work on protein foods.

The auxiliary components of the drugs are also not harmful to humans and, in addition, pancreatin is not absorbed into the bloodstream, but is excreted from the body with feces.

Considering all of the above, gynecologists and therapists often recommend Mezim to eliminate digestive problems in pregnant women. In each case of prescribing a drug during pregnancy, the doctor bears full responsibility for his decision.

Therefore, you should only take the medicine if you completely trust your doctor.

It is recommended that pregnant women take the tablets while sitting or standing, because in the lying position the tablet moves more slowly through the digestive canal and may begin to dissolve before reaching the stomach.

Use of Mezim during breastfeeding

Use during breastfeeding is possible according to doctor's orders, which are made taking into account the ratio of the expected therapeutic effect for the mother's body and the potential risks for the infant.

What do you need to know before you start taking Mezim?

Indications for taking Mezim are:

  • cystic fibrosis (hereditary disease of the pancreas);
  • pancreatitis;
  • pancreas cancer;
  • people who have had surgery to remove all or part of the pancreas or stomach;
  • treatment of steatoria (a condition in which large amounts of fat are excreted in feces);
  • lack of pancreatic enzymes in newborns, to improve digestion.

To make sure Mezim is safe for you, tell your doctor if you have any of the following contraindications :

  • allergy to the active substance, auxiliary components, pig meat;
  • gout;
  • asthma;
  • acute pancreatitis or relapse of chronic pancreatitis;
  • kidney diseases;
  • pregnancy and breastfeeding.

Reviews of Mezim Forte

Reviews about Mezim Forte are either good or very good. The product is an excellent remedy for heartburn , bloating , early satiety, belching , nausea , and flatulence .

Most people believe that the drug is indispensable for those who like to eat delicious food, confirming this with the words: “... If your stomach hurts after overeating, take Mezim, and the feeling of heaviness goes away.”

The appearance of lightness in the stomach after taking the pill most likely served as a prerequisite for the emergence of the opinion that the drug could be used for weight loss. However, this is nothing more than a myth - Mezim is not intended to treat obesity and eliminate excess weight problems, it only helps to normalize digestion and facilitate the work of the life-sustaining system.

Preventing recurrent heartburn

There are some rules that can help reduce the symptoms of heartburn.
To prevent heartburn, you need to follow simple rules that will help reduce the load on the stomach and reduce unpleasant symptoms:

  • Small portions of food 4-5 times a day.
