Methods for calculating gestational age

In gynecological practice, the obstetric method of calculation is used - the gestational age is calculated from the first day of the last menstruation. The embryonic method differs from the obstetric method by 2 weeks, and calculates the gestational age from the day of ovulation, approximately from the middle of the cycle.

Some women count their pregnancy from the day of their missed period, when they received visual confirmation of their pregnancy.

If we take all these methods into account, then the fifth week from conception is equal to the seventh obstetric week of pregnancy. The fifth week from the day of missed menstruation is equal to the ninth week according to the obstetric method. The information below is mostly oriented from a medical point of view.

Condition of the uterus and endometrium

The size of the uterus at the 4th week of pregnancy practically does not increase; it is still located within the boundaries of the small pelvis. Its structure changes - the uterus becomes swollen, the tissues in the isthmus area soften, which gives it slight mobility. Both the uterus and its cervix become bluish due to the influx of blood. A mucous plug begins to form in the cervical canal, protecting the fetus from bacterial infection from the outside.

The endometrium thickens, its structure changes - new blood vessels sprout, the number of glandular cells increases, which subsequently transform into the placenta. At this stage, such changes ensure the viability of the embryo, its respiration and nutrition. Due to the large number of blood vessels, the endometrium becomes bluish in color. Its thickness at 4 weeks, calculated using the embryonic method, is about 2 cm.

Accurate determination of pregnancy

Now the pharmacy test will show the exact result.

In order for the study to be carried out in accordance with the manufacturer’s recommendations, there are several rules:

  • Pregnancy determination is carried out at any time of the day, but the morning test is most accurate.
  • The second strip may not appear immediately, but after a short period of time, so it makes sense to temporarily put the test strip in the box.
  • It is advisable to repeat the result obtained after 1-2 days, you can do this twice.

Ultrasound at 5 weeks of pregnancy

Indications for examination at this stage of pregnancy are a history of miscarriages or ectopic pregnancy, the appearance of atypical discharge or severe abdominal pain. By analyzing ultrasound data, the doctor will accurately determine the presence of pregnancy, the number of embryos, the place of their development in the uterine cavity, and the condition of this organ.

It will most likely not be possible to examine the embryo in detail. In the photo it will look like a light dot. Precise modern equipment is able to determine this week the diameter of the amniotic sac and the length of the embryo from the coccyx to the crown - KTR, or coccygeal-parietal size. These parameters will be analyzed by a doctor until 20 weeks of pregnancy, then the fetus will be measured along its entire length - from the heels to the crown.

What happens to the fetus at this stage?

According to the obstetric method, at 4 weeks the zygote, or fertilized egg, begins to rapidly divide. It produces many cells with different structures and functions. Actually, it is not yet a fruit. The collection of cells is called a morula, or blastocyst. In shape, it resembles a multi-chambered berry, not yet attached to the endometrium of the uterus. It is difficult to name the exact parameters of the blastocyst, since it is continuously growing.

Approximate parameters of the future embryo:

  • Weight – 0.5 mg;
  • Height – 0.36-1 g.

In figurative language, it is comparable in size to a poppy seed.

If we calculate the gestational age using the embryonic method, at week 4 the embryo has already established itself in the uterine cavity and continues intensive development.

At this stage, it is a germinal disc and consists of 3 layers:

  • Primary ectoderm is the basis of skin, hair, tooth enamel, eye lenses, nervous system cells;
  • Mesoderm is the basis for the formation of future organs, skeleton, muscle tissue, circulatory, excretory and reproductive systems;
  • Endoderm is the basis of the respiratory and digestive systems, liver, thyroid and pancreas.

The future fetal head currently looks like a small droplet. The placenta has already begun to form. While it is still very primitive, it consists of 2 layers. The tissues of the future placenta are already connected to the tissues of the uterus, which makes it possible to provide the embryo with nutrition and oxygen.

The following extraembryonic organs are responsible for nutrition:

  • The chorion is a prerequisite for the real placenta, which will form until the 12th week; from the 4th week, a vascular network begins to form at the point of contact of the embryo with the endometrium of the uterus;
  • Amnion is a kind of “packaging” of the embryo, a prerequisite for the amniotic sac, which protects the fetus from physical impact and drying out;
  • The yolk sac is a supply of nutrients for the embryo, the basis of hematopoiesis until 7-8 weeks of pregnancy.

Without the yolk sac, the existence of an embryo is impossible; if its functions are impaired or there is congenital pathology, a miscarriage occurs. When performing an ultrasound, the doctor determines the quality of pregnancy and the viability of the embryo based on the condition of the yolk sac.

According to research, it turned out that male genes are responsible for the formation of extra-embryonic organs. We can say that the function of protection and provision is inherent in a person even before birth.

By the end of 4 weeks, blastogenesis, the initial stage of embryonic development, ends.

Hormones and tests

At week 5, the corpus luteum of the ovary produces the most important hormones of pregnancy - estrogen and progesterone. A reduced concentration of progesterone can put pregnancy at risk of termination, since this hormone prevents the uterus from contracting, relaxing its muscle layer. The chorion, or fetal membrane, actively produces a specific hormone - human chorionic gonadotropin.

Determining hCG in urine or blood serum will confirm the presence of pregnancy with 100% accuracy. If a repeat blood test shows a decrease in hCG concentration after its preliminary increase, this may be a sign of an ectopic pregnancy.

If a woman is facing the threat of termination of pregnancy, or she decides to register with the antenatal clinic, the obstetrician-gynecologist will prescribe the following tests:

  • General blood analysis;
  • General urine analysis;
  • Determination of blood group and Rh factor;
  • Test for AIDS, syphilis, hepatitis;
  • Analysis to determine blood clotting (coagulogram).

If there is a threat of miscarriage, a quantitative urine test for hCG is added.

Pregnancy management after IVF

Proper gestational support reduces the likelihood of spontaneous abortion and abnormal fetal development. To prevent complications, it is advisable to be observed by specialists at the clinic where the IVF procedure was performed within 6-8 weeks after conception.

During the first trimester of pregnancy, patients should undergo regular blood tests to determine the level of:

  • human chorionic gonadotropin;
  • estrogen;
  • progesterone.

A low concentration of steroid hormones is the basis for corrective hormonal therapy. It should be understood that an ordinary obstetrician-gynecologist in the antenatal clinic does not have sufficient experience in managing patients who have undergone IVF. If the risk of miscarriage increases, the fertility specialist will recommend undergoing additional examination or hospitalization.

How the fetus develops - size and functioning of organs

At this stage of pregnancy, the tiny embryo passes an important milestone - it begins to separate from the extra-embryonic organs. Outwardly, it looks like a small tadpole or a tube rolled into an arc. The rudiments of limbs and fingers and toes are already visible, a head with eyes, a mouth opening, and pits instead of ears is emerging. It is at the 5th obstetric week that the laying of reproductive organs and germ cells occurs. Depending on the sex of the embryo, which was determined at 4 weeks, these are future sperm or eggs.

Layers of embryonic tissue to form organs:

  • Ectoderm (outer layer) - medullary, or neural tube, giving rise to nerves, brain, spine, as well as skin, hair, tooth enamel, sweat and mammary glands;
  • Mesoderm (middle layer) - heart, circulatory system, bone skeleton, muscles, cartilage, subcutaneous tissue;
  • Endoderm (inner layer) – lungs, intestines, liver, pancreas and thyroid gland, urinary system.

The fetus has its own blood type, and its circulatory system begins to develop. The embryo, which is only 2 mm long, already has a beating heart. It will be possible to clearly hear the heartbeat only at the 7th week of pregnancy, while uneven and irregular beats are still indistinguishable by the most sensitive equipment. The formation of the rudiments of the liver and pancreas, larynx, and trachea has begun.

Significant changes occur in the fetal neural tube - the prototype of the future central nervous system, brain and spinal cord. Its middle sections close, and along the neural tube, areas of future muscle and bone tissue are formed and differentiated. A sufficient amount of folic acid received by the expectant mother plays a major role in the proper formation of the nervous system. Its deficiency in the future may affect the development of the child’s intellectual and motor functions and cause developmental defects.

Fruit size:

  • Length – 1.5-2 mm;
  • Weight – 1-3 g.

At the 5th obstetric week, the placenta is formed, through the umbilical cord and the birth place of which the fetus will receive nutrition and excrete waste products until birth.

It is important to maintain a positive emotional attitude, because there is already a connection between mother and child, for whom positive emotions only benefit. There is research data on the development of wanted and unwanted children, for whose mothers the news of pregnancy became negative. Desired children, starting from the period of intrauterine development, developed more actively, had a minimum of developmental pathologies, and childbirth was less traumatic.

Feelings of a pregnant woman at 5 weeks

During this period, significant mood swings can be felt - from euphoria over the most insignificant occasion to irritability over trifles, turning into a depressive state. Each woman’s physical condition is individual - some do not notice any changes, while others constantly want to sleep, get tired quickly, and find it difficult to concentrate.

With an unstable menstrual cycle, some women may still be unaware of their “interesting” situation, attributing the delay in menstruation to its fluctuations.

However, most often, there are signs of pregnancy that are difficult to miss:

  • Taste preferences;
  • Loss or increase in appetite;
  • Nausea and vomiting;
  • Increased salivation or “runny nose during pregnancy”;
  • Swelling of the breasts and darkening of the areolas of the mammary glands;
  • Acute perception of smells;
  • Frequent urination;
  • Clear identification of a strip of skin from the navel to the pubis.

How does mom feel?

The unpleasant sensations that the expectant mother experiences during this period can cause anxiety. But even the strangest of them can be a completely normal manifestation of pregnancy. And if you make some changes to your daily routine and diet, then such sensations will not interfere with enjoying the anticipation of the birth of your baby.

Early toxicosis.

The restructuring of the body during pregnancy affects all organs, including the digestive and nervous systems. Hence the periodic episodes of nausea and/or vomiting that the expectant mother experiences. Normally, such urges should occur no more than 4 times a day. If nausea and vomiting become more frequent and do not allow a woman to eat normally, you should inform your gynecologist about this. This is necessary, since with excessive toxicosis, the pregnant woman’s body loses important substances and fluid, and frequent nausea does not make it possible to compensate for the losses.

Increased fatigue.

During pregnancy, the expectant mother spends energy not only on herself, but also on the baby growing in her body. And its energy needs are quite high: every second and minute it grows and develops, this requires a lot of resources. It is not surprising that a pregnant woman often experiences bouts of drowsiness and a desire to rest.

Frequent urination

. Although the uterus has increased slightly in size, it nevertheless already puts pressure on the bladder. For this reason, the expectant mother notes a more frequent urge to urinate.

Bloating and constipation

. Slowing peristalsis is the result of progesterone and the main cause of gas accumulation and constipation in the intestines. But if the expectant mother has severe toxicosis, then a delay in bowel movements may be so-called starvation constipation.

Mood swings

. Hormones are very important for the proper course of pregnancy. But they are the ones that cause strange mood swings. Within a short time, a woman can repeatedly move across the entire “scale” - from a depressed and even devastated state to a feeling of endless happiness and joy.

New taste preferences.

A pregnant woman's body is very sensitive and works in tandem with the growing baby. It is not recommended to make drastic changes to your diet; you still need to listen to your body. And if you suddenly want an unusual product that definitely won’t cause harm, then you shouldn’t deny yourself and your child.

Abdominal cramps.

Such sensations can be caused by both digestive disorders and the attachment of the fetus to the wall of the uterus.

Toxicosis of pregnancy

Early toxicosis does not necessarily have to manifest itself as debilitating vomiting or constant nausea. Most often, these are minor manifestations, intolerance to odors or certain foods. This category includes protein foods, fried foods, and fast food. If the last two categories are not recommended for the nutrition of pregnant women, then a lack of protein can negatively affect the development of the fetus.

Meat, poultry, eggs can be temporarily replaced with nuts and legume dishes. Another option is to introduce proteins in small quantities so that the smell of the meat, for example, is covered by the aroma of the side dish. Milk may also be included in unacceptable foods, although it is an important source of calcium. In this case, it is worth trying to replace it with fermented milk products, or vegetable and fruit juices, salads.

Nutrition rules for toxicosis:

  • It is advisable to have breakfast without getting out of bed;
  • The morning meal should contain juice, fruit drink, fruits such as banana or apple;
  • After breakfast, you need to lie down for a while without getting out of bed immediately;
  • It is advisable to carry a light “snack” with you in the form of a cookie or a piece of cheese, so that you can consume it if symptoms of toxicosis appear.

Uncontrollable vomiting in early toxicosis is a reason to consult a doctor and be treated in a hospital. Nausea with early toxicosis can last up to 11-12 weeks in a singleton pregnancy, and up to 14-16 weeks in a multiple pregnancy.

Another striking symptom of early toxicosis is heartburn, which occurs due to decreased smooth muscle tone. This symptom is due to hormonal changes, the effect of progesterone on the tone of the uterus and all internal organs. The muscles of the esophagus and stomach weaken, and a small amount of gastric juice refluxes into the esophagus. This symptom disappears at 4 months of pregnancy.

Lifestyle of an expectant mother

Over the next few months, the pregnant woman needs to take care not only of the baby, but also of herself. After all, the expectant mother should provide conditions for the birth of a strong and healthy child and maintain her own health. Therefore, you should adjust some aspects of your life.

Nutritional features.

For those who consider a pregnant woman’s diet to be something special and requiring a serious investment of time and money, there is good news. In fact, such a menu is no different from a regular healthy diet. You should limit fried, smoked, spicy foods, alcohol in your diet, and also minimize the consumption of sweets and baked goods. In return, you need to introduce as many fresh vegetables, fruits, berries and herbs into your diet, give preference to lean meat and fish, and do not forget about fermented milk products and whole grain porridges and bread. When cooking, it is recommended to use stewing, boiling and steaming.

Taking vitamins.

It is best to take vitamins and minerals necessary to maintain pregnancy and the proper development of the baby before conception. This way the body will prepare as fully as possible for bearing a child. But during pregnancy, vitamins and minerals are no less important. Which ones should be taken will be advised by the doctor who is involved in pregnancy management.

Vaginal discharge - normal and pathological

High levels of progesterone in the 5th week of pregnancy do not allow other eggs to develop, and menstruation does not occur, which is a rejection of the endometrium. Any bleeding during this period is a reason for medical consultation. Some women experience slight red or brown discharge during pregnancy. If the threat of ectopic pregnancy is excluded, this discharge may indicate a lack of progesterone. Normally, at the 5th week of pregnancy, vaginal discharge looks like thick, transparent mucus, which is released a little more than normal.

Atypical discharge:

  • Curdled, in the form of flakes;
  • Yellow or greenish;
  • With an unpleasant odor.

It is quite possible that these are signs of thrush, or vaginal candidiasis, which has arisen against the background of hormonal changes and decreased immunity. She is treated on an outpatient basis together with her sexual partner. If thrush is ruled out, such vaginal discharge may be a sign of a sexually transmitted disease and requires immediate treatment.

When to see a doctor?

If you feel normally, you can still postpone a visit to the doctor. If the test result is positive and there is nagging pain, signs of early toxicosis, or atypical discharge, you should consult a doctor.

Possible actions of a gynecologist if a woman feels well at the 4th obstetric month:

  • Taking anamnesis;
  • Visual examination, examination on a gynecological chair;
  • Prescription of vitamin-mineral complexes containing, among other components, folic acid.

Signs of disease and a history of miscarriages require specific treatment. Women who have problems conceiving and become pregnant using IVF or ICSI are under the supervision of a doctor from the first days of pregnancy.

Abdominal and lower back pain

At 5 weeks of pregnancy, the uterus may periodically be in good shape. Women feel this condition in the form of a nagging pain in the lower abdomen. If such manifestations occur frequently or are accompanied by bloody discharge, the threat of miscarriage should be ruled out. If you consult a doctor in a timely manner, the pregnancy can be saved.

Another cause of pain in the lower abdomen may be an enlarged uterus. It changes its location, becomes spherical, puts pressure on internal organs: intestines, bladder. A shift in the center of gravity causes lower back pain. During this period, it is advisable to rest more often in a lying position and not wear high-heeled shoes.

Pregnancy hormones have an effect on internal organs, in particular, the outflow of bile from the gallbladder is disrupted, which can also cause abdominal pain. The addition of temperature to pain in the lower back indicates the onset of pyelonephritis. If the pain is localized on the right or left side of the abdomen, it is possible to diagnose an ectopic pregnancy; below in the projection of the uterus - a frozen pregnancy. These conditions require immediate examination and treatment in a hospital.

How does a woman feel at 4 weeks pregnant?

If the pregnancy was unplanned and the woman does not suspect that she is pregnant, it is possible that she does not experience any changes in well-being. But this does not happen often; in most cases, the woman’s mental state changes, she becomes extremely emotional, touchy and whiny, irritable.

Additional sensations:

  • The perception of the smells and taste of familiar food has changed;
  • The figure became somewhat heavier due to hormonal changes;
  • Swelling of the limbs appeared;
  • Nausea occurred, and in some cases vomiting (early toxicosis);
  • Nagging pain appeared in the lower back and lower abdomen;
  • Increased frequency of urination;
  • Vaginal discharge takes on the character of thick mucus;
  • Increased sensitivity of the mammary glands appeared.

It is quite possible that due to early toxicosis the woman will not recover, but, on the contrary, will lose weight. There is no need to worry about this if there are no other symptoms other than loss of appetite.

Frozen pregnancy and miscarriage

The condition when the embryo stops its development, and placental abruption and rejection of the fertilized egg does not occur, is called frozen pregnancy. It is accompanied by bloody or bloody discharge. A frozen pregnancy can pose a danger to the life and health of a woman, as there is a high risk of infection of the pelvic organs. Once the diagnosis is confirmed, the doctor takes measures to remove the fertilized egg.

A non-viable fetus with developmental defects at 5 weeks can cause a miscarriage or spontaneous abortion.

Causes of miscarriage:

  • Women's hormonal imbalances;
  • Genetically determined diseases;
  • Rh factor;
  • Infections suffered by a woman;
  • Somatic diseases incompatible with pregnancy;
  • Emotional overload, stress;
  • Side effects of medications;
  • Physical exercise;
  • History of abortion;
  • Overheating in a bathhouse, sauna.

Women with elevated levels of male sex hormones, excess hair growth and a male-type physique, as well as overweight women are more likely to be at risk.

Hormones at 4 weeks

The chorion (embryo membrane) produces hCG - human chorionic gonadotropin. It prevents the rejection of the embryo, which represents a symbiosis of its own and foreign cells for the body, produces steroids, determining the sex of the unborn child, and supports the functioning of the corpus luteum. HCG levels increase daily until 7-8 weeks of pregnancy, then they begin to decrease.

Before the formation of the placenta, the corpus luteum produces progesterone, which plays an important role in the normal course of pregnancy.

Functions of progesterone:

  • Ensuring the growth of the uterus;
  • Reducing the frequency and intensity of uterine contractions, preventing miscarriage;
  • Growth of subcutaneous tissue to nourish the embryo;
  • Prevention of rejection of a foreign organism;
  • Development of individual fetal tissues;
  • Preparing the mammary glands for breastfeeding.

The average amount of progesterone in the blood of a pregnant woman at week 4 is 18 nmol/l. Before the study, physical and mental stress and taking hormones are not recommended.

Abortion

An unwanted pregnancy can be terminated using the following methods:

  • Medical abortion – taking drugs to induce a miscarriage, used up to 6 weeks of pregnancy;
  • Mini-abortion, or vacuum abortion - termination of pregnancy using vacuum suction under ultrasound control, used up to 5 weeks of pregnancy;
  • Surgical abortion - curettage of the uterine cavity, used up to 12 weeks of pregnancy.

For a period of 5 weeks, the first two methods are most often used. Vacuum abortion is performed under the influence of an anesthetic drug for 10 minutes. Rehabilitation after a mini-abortion occurs quite quickly, with a minimum of complications.

Medical abortion is performed under medical supervision and is considered the safest method today.

Diseases during the 5th week of pregnancy

    – a common occurrence in pregnant women, its type – vasomotor rhinitis, occurs due to hormonal changes in the nasal mucosa. It goes away on its own after childbirth. The viral type of rhinitis cannot be treated with vasoconstrictor drops due to the risk of spasm of the placental capillaries and the occurrence of fetal hypoxia. The nose is washed with herbal infusions and preparations with sea water.
